When people choose a pillow, firmness is usually treated as a comfort preference — “I like something soft” or “I prefer firm.” This is the wrong frame. Pillow firmness is a structural specification that should match your sleep position, body weight, and shoulder width. Choosing the wrong firmness creates the same cervical misalignment as choosing the wrong loft height.

Why Firmness Matters

Firmness determines how much the pillow compresses under the weight of your head. An adult head weighs approximately 5–6kg. A soft pillow compresses significantly under this load; a firm pillow resists and maintains its loft. The question is: how much compression is appropriate for your position?

  • Side sleepers need medium-firm to firm: The pillow must resist compression enough to maintain the full shoulder-to-ear height. A soft pillow compresses under the head, reducing effective loft by 30–50% — which eliminates the support side sleepers need most.
  • Back sleepers need medium to medium-soft: The head rests more centrally on the pillow rather than pressing down from the side. Some compression is acceptable and can help cradle the natural cervical curve. Too firm and the pillow pushes the head forward.
  • Combination sleepers need medium: A balance that does not collapse for side sleeping but does not push back too hard for back sleeping.

Lighter people and narrower frames can often use a slightly softer pillow because the compression from their lighter head weight is less severe.

A practical test: lie on your side on your current pillow. Place your hand flat on the mattress at neck level. If your neck is below your hand’s level (drooping), the pillow is too soft or too flat. If it is above your hand’s level (arching upward), the pillow is too firm or too thick. You want your neck and hand to be at the same level.

The Firmness Problem With Memory Foam

Memory foam adds a complication: it changes firmness with temperature. Cold memory foam is firmer; warm memory foam softens. This means a memory foam pillow may feel too firm initially and then reach its correct firmness as it warms to body temperature over the first few minutes of sleep.
